Role Overview
We are seeking a dynamic and innovative individual who will work enthusiastically to deliver recovery oriented, client and family centered and comprehensive services that meet client needs, ensure client safety and work within a quality improvement framework to identify opportunities for enhancing quality and quantitative outcomes of the services. Client and family centered care is guiding principle at Cota that helps improve decision making processes, health outcome and client experiences. The successful individual will embed this guiding principle into their work.
About the Team
We require an experienced Supportive Housing Worker to work as a team member in our Modular Housing program at 321 Dovercourt Road. This site is part of the City’s Phase 1 Modular Housing Initiative. This site provides permanent affordable housing with supports to people who are exiting chronic homelessness.
Each tenant has his/her own bachelor apartment within the building, access to a meal program offered by the building operator (The Neighborhood Group), and access to onsite supports provided by Cota’s supportive housing team.
Role Description
The Supportive Housing Worker supports tenants to maintain their housing, develops independent living skills, and pursues personal recovery and wellness.
Summary of Duties:
· Assist with the development of client’s support/goal plans
· Share responsibility with the team for implementing appropriate housing supports that assist tenants in pursuing/achieving their goals
· Assist tenants with tenancy related matters such as rent payment, reducing high risk behaviors, engagement within the life of the tenant community
· Support tenants with:
o Reminders relating to appointments, accompaniment to appointments
o Establishing and/or maintaining personal wellness routines
o Managing and supporting clients with behavioral needs
· Support the maintenance of safe/clean environment:
o Directly assist with maintaining the apartment
o Continually assessing the environment for safety and security issues
· Plan and facilitate group activities/events which foster tenant engagement/participation, the development of social networks and skill acquisition
· Documentation and data entry, document all tenant contact/interactions in the appropriate client chart according to Cota documentation policies
· Participate in team meetings; provide input and ideas for improving services and efficiencies and ongoing program planning.
Summary of Qualifications:
· A minimum of a Community College Diploma in a field relevant to the target population. Higher level post-secondary education will be considered an asset
· A minimum of 2 years direct or transferable experience suitable to working effectively with the relevant target population
· Knowledge of community resources relevant to the target population being served and/or demonstrated ability to search for applicable resources/information
· Ability to design and implement group activities and facilitate group meetings
· Good crisis management and dispute resolution skills
· Good interpersonal and communication skills (verbal and written)
· Basic computer literacy (Word, E-mail, Internet)
· Experience in community mental health and/or in supportive housing settings
· Basic understanding of different psychiatric conditions
· Experience with the development of tenant recovery plans
· Experience working with developmentally delayed or addictions is preferable
Other Requirements:
· Adhere to Cota’s COVID-19 Vaccination Status Disclosure policy
· Proof of Full COVID-19 Vaccination status is required as a pre-condition of employment
