Job Summary and Requirements

This posting is for one (1) 2-year Term working 75 hours biweekly, Monday to Friday, with the option to work reduced hours (60 hour minimum) for work life balance.

This will also be used to create an eligibility list for future perm/term vacancies, AOC, casual.

The Social Worker provides professional social work services to residents/clients in HOME CARE within the scope of practice, standards of practice, organizational policies and procedures, and competencies.

The responsibilities for this position include working in an interdisciplinary team environment to provide psychosocial assessments and social work services for all residents/clients and their families, including and providing care coordination, support, responding to issues of abuse, neglect & self-neglect, advocacy services and providing education & support to palliative and complex residents/clients and their families for the development of Advanced Directives and end of life planning. You will participate in the development and implementation of programs, and perform other related duties.

Essential Qualifications

Bachelor of Social Work
Experience providing case management as a Social Worker
Experience completing assessments

Desired Knowledge, Skills and Suitability

Candidates should have and may be assessed on:
Strong ability to collaborate and work within a multi-disciplinary team
Excellent organizational, interpersonal and conflict management skills
Effective oral and written communication skills
Strong skills working with various cultural backgrounds
Ability to take a reflective approach to your practice

Conditions of Employment: TB Screening, Vulnerable Sector RCMP security clearance, CPR Level C (within the last 6 months) valid Class 5 Yukon Driver’s Licence, WHMIS 2015 and current registration with a Canadian Social Work regulatory body

Job Requirements: Exposure to pets, exposure to second-hand smoke, latex products, flowers and/or plants and other irritants while in client homes or within facilities. Travel within Whitehorse and surrounding areas is required
