Copernicus Lodge is a not-for-profit, charitable and ethno-specific organization made possible through the efforts of the Polish Canadian community and the vision of Father Michael Smith, OMI. Located in the heart of the Roncesvalles community in the heart of west–end Toronto, we provide a continuum of care in a predominantly Polish, Roman Catholic environment to meet the changing needs of our seniors. The Lodge is comprised of an Adult Day Program, 200 independent, affordable self-care apartments with a Supportive Housing program, and a 228 bed Long Term Care home.

We are seeking a dynamic and innovative individual to step into our open Registered Nurse role in our Long Term Care Home, who will work enthusiastically in a client and family-centered and comprehensive services environment that meets client needs. They will ensure resident safety and work within a quality improvement framework to identify opportunities for enhancing the quality of the care provided to our residents.

The Registered Nurses (RNs) are responsible for helping lead a skilled team of Personal Support Workers (PSWs) and Registered Practical Nurses under the guidance of the Director of Care and Assistant Director of Care.

KEY ROLE DUTIES:

  • Supports the plan of care of residents who are medically stable, with predictable outcomes
  • Asses, plans, implements and evaluates the care of residents who have become unstable, and/or have unpredictable outcomes;
  • Documentation;
  • Medication administration;
  • Supports RPN and unregistered staff in care delivery;
  • Revises assignments that reflect changing resident care needs;
  • Consults with external care providers – such as – but not limited to - attending physicians, mental health and geriatric practitioners;
  • Ensures that documentation captures the visitation and discussion regarding resident/family care;
  • Family support and education;
  • Ensure that computerized care plans are current
  • Collaborates with PSW and RPN staff to ensure the accuracy of MDS, care plans, and other assessments
  • Ensures that RAI requirements are met, including completing assigned MDS
  • Participates in audits as required – including hand hygiene, and documentation.
  • Other duties as assigned by DOC/ADOC

QUALIFICATIONS

  • Member in Good standing of the College of Nurses of Ontario
  • Post-secondary Certification or training in gerontology or related fields, is desirable
  • Current CPR Certification.
  • Excellent interpersonal and communication skills – ability to speak fluently and read/write in English
  • Speaking knowledge and understanding of Polish language and customs is a definite asset. Failing this, a genuine interest and effort to learn basic Polish words/phrases to assist in providing resident care and services;
  • Expertise in using care planning software (e.g. PointClickCare) to document resident care
  • Minimum one year experience as an RN in Geriatrics.
  • Demonstrated leadership ability
  • Excellent physical assessment skills, critical thinking skills and problem solving skills.
  • Demonstrated understanding and commitment to resident and family-centred care.
  • Demonstrated ability to organize resident care demands and to set priorities for care needs
  • Demonstrated ability to engage and educate residents and families.
  • Demonstrated commitment to ongoing professional development and learning.
  • Excellent computer skills and knowledge of electronic care planning and delivery.
  • Demonstrated ability to work collaboratively within an interprofessional team as well as independently.
  • Excellent attendance record and ability to maintain same.
  • This position is the In-Charge Nurse for the home in the absence of the DOC and ADOC.
  • Able to work shifts/flexible hours including weekends.
