We currently have a Regular Full-Time opportunity for a Nurse Practitioner in the Veterans Program Primary Responsibility: Dementia Care Department at our Bayview Campus.

Hours of Work:

Weekdays & Weekends: Days; 8 hours; Flexibility in hours will be required


Summary of Duties

  • The Nurse Practitioner is an advanced practice role designed to contribute to the quality of patient care through practice, research and education. An emphasis of this role is direct patient care, including the daily clinical management of a defined group of geriatric dementia patients and the coordination of interprofessional patient care.
  • Provides leadership to identify, plan, and implement initiatives to advance best practices for dementia and gerontological care.
  • Acts as an expert resource to assist in the transfer of knowledge and skills related to dementia care, and participates/leads related research activities.
  • Participates in ongoing program planning and evaluation of clinical outcomes.
  • Reports to the Veterans Program Operations Director and indirectly to the Veterans Program Chief.

Qualifications/Skills

  • Current College of Nurses of Ontario Certificate of Competence
  • Master’s degree with Nurse Practitioner Certification
  • Demonstrated clinical expertise in gerontology and dementia care
  • Current BCLS
  • Previous experience as a Nurse Practitioner an asset
  • Gerontological Nurse Certification an asset
  • Advanced clinical judgment and the ability to manage residents presenting with moderate to advanced dementia and other co-morbid health conditions
  • Program planning and evaluation skills
  • Demonstrated research abilities
  • Demonstrated professional leadership and ability to be innovative and creative
  • Highly organized with strong communication and interpersonal skills.
