Cambridge North Dumfries Ontario Health Team Interim Primary Care Lead

Short-Term Contract - May-September 2023 (5 months)

The Cambridge North Dumfries Ontario Health Team (CND OHT) is committed to engaging primary care clinicians (physicians and nurse practitioners) in healthcare transformation. Primary care is the foundation of the health care system and has a unique perspective on existing challenges and pain points that must be addressed to enable transformation and integration of care.

The Ministry of Health has communicated the commitment to support collaboration and engagement of primary care partners through the creation of primary care networks. As per recent announcements, there is an intention that every Ontario Health Team will include a group of primary care providers organized in a network to be part of decision-making and to improve access to care for patients.

What We are Looking For:

The Cambridge North Dumfries OHT is recruiting for up to two interim primary care co-leads to increase and improve primary care involvement in the planning, implementation, and decision-making of the Cambridge North Dumfries Ontario Health Team. The CND OHT Primary Care Lead will report to the CND OHT Transformation Lead and will be supported by the CND OHT staff support team. Deliverables and responsibilities for this role will include:

  • Act as the point of contact for the Ontario Health West Primary Care Lead to disseminate key Ontario Health updates to primary care colleagues across Cambridge North Dumfries.
  • Expand the CND OHT primary care distribution list to ensure reach to all primary care across Cambridge North Dumfries
  • Co-design with primary care colleagues (both members and non-members of the CND OHT) a process to identify a CND OHT Primary Care Lead, and carry out the process.
  • Represent the CND OHT locally, regionally and provincially as the Interim CND OHT Primary Care Lead.

In addition to these key deliverables, the CND OHT Interim Primary Care Lead will support:

  • Improving primary care involvement in CND OHT implementation and decision-making via a co-designed approach with primary care colleagues.
  • Establishing readiness across CND primary care for the development of a primary care network. This may involve hosting engagement sessions, engaging in learning modules, preparing readiness assessments and other activities.
  • Collaborating with the CND OHT staff support team to strengthen primary care engagement in key CND OHT activities and projects related to connecting care, quality and digital health.

Key Competencies:

System thinker with an understanding of clinical leadership in health system integration, design, planning and implementation.

  • Excellent communicator, strategic thinker and team player, with a passion for health system improvement.
  • Self-directed and highly motivated, with the ability to navigate operational ambiguity
  • Established track record of influencing, achieving consensus and driving change.

Qualifications & Experience:

  • A Cambridge North-Dumfries practicing primary care physician or nurse practitioner.
  • Knowledge of provincial health care structures, processes, funding mechanisms and current priorities is strongly preferred.

Compensation is based on CND OHT primary care leadership remuneration rates. Time allocation estimated to be equivalent to 6-8 hours per week, spread throughout the week. Additional opportunities for involvement possible based on need and interest.
