Description: The Social Worker II (SW) provides services to clients and families including psychosocial assessment, crisis intervention, education, treatment, advocacy, resource finding, consultation, and discharge planning. The SW II is responsible for the provision of services in accordance with established standards, and the Mission, Vision, Values, policies and procedures of Covenant Health. This position is covered by the terms and conditions of the HSAA Paramedical Professional/Technical Collective Agreement.

Required Qualifications:

Required Qualifications: Graduate of a Social Work program or recognized equivalent required. Active registration with the Alberta College of Social Workers (ACSW) required.

Additional Required Qualifications:

Bachelor's degree in social work required. Recent 3-5 years' experience with seniors diagnosed with Mental Health and Addictions living in the community. Position requires significant periods of time sitting while interviewing patients/families on a phone or computer. Requires inter site and/or inter community travel and in home community visits. Frequent need to perform multiple tasks in a fast-paced environment as well as the frequent need to concentrate on precise and detailed work. Required to participate in sensitive and challenging patient situations and expected to deal with the public in diverse situations. Basic Life Support Healthcare Provider (Level C) required.

Connect Care Training. NCI. Capacity Assessor training considered an asset.
