Qualifications:

• Bachelor's Degree in Social Work from a recognized University program
• Must be a registered member of Ontario College of Social Workers and Social Service Workers (OCSWSSW)
• Have at least 2 years of relevant experience in social work or other related field, preferably in health care/long term care
• Possess sound knowledge of social work methods and techniques

Duties and Responsibilities

• Works in conjunction with the Behaviour Supports Ontario (BSO) program
• Liaises with Social Service Workers in the community for the purpose of transitioning a resident back to the community. Liaises with community Mental Health Care services, offering resources and assisting with the transition of residents who have a mental health diagnosis
• Assists and supports new resident/substitute decision maker with admission into the Home. Provides one-to-one counseling where necessary and may refer to external resources for further counseling for the purpose of facilitating the resident’s adjustment to the Home, supporting psychosocial well-being, and supporting quality of life dimensions
• Provides counsel and therapy to assist residents and or family members to develop skills to deal with and resolve their social and personal problems, and an ad hoc or planned intervention basis. May also develop Support Group services for residents and family
• In combination with a PIECES/GPA trained registered team member, assists with completing cognitive functioning tests such as the MMSE an or MOCA. Referrals are made by the physician and or through the cognitive performance scale identified in the MDS RAI system
• Attends the multi-disciplinary care conference with resident and family members as required by the Resident being reviewed.
• Facilitates internal resident transfers in collaboration with the resident/substitute decision maker, facility staff and other residents
• Supports the End of Life Program to include the facilitation of the Celebration of Life .
• Provides orientation and education to the team members of the Home in regard to the creation of a therapeutic environment; congenial atmosphere; healthy interpersonal relationships; enhancing staff understanding of the normal aging process and the Resident Bill of Rights; heightening insight into early recognition and intervention of psychosocial problems; and promoting referral of residents to the Social Work services
• Provides monthly reports on key indicators to the Administrator
• Evaluates the effectiveness of counseling and social service programs
• Performs any other related duties and projects as assigns by the Administrator
• Facilitates Spiritual and emotional support for the Resident and team member population as required.
